If you are coming to The Gentle Place for a massage for the first time, we ask that you plan to arrive 10 minutes early to fill out your health history paperwork and make sure we can start on time. You can also fill out the paperwork in advance by downloading it here to print at home and bring with you.
Additionally, if you are coming in for a PreNatal Massage, we will also need you to fill out our PRENATAL Informed Consent Form. If you have a high-risk pregnancy, a note from your doctor will be required.
We welcome offering massage therapy to children and minors, but their guardians will need to remain at the office during their treatment, and will need to fill out our Minor Consent form.
